Notice of Privacy Practices (HIPAA)

As a covered entity under HIPAA, we protect the privacy of your protected health information (PHI). We use and share PHI for treatment, payment, and health care operations, and as otherwise permitted or required by law. You have rights over your PHI, including the right to see and copy your records, request changes, and ask for an accounting of disclosures. California Confidentiality of Medical Information Act (CMIA)

Your medical information is also protected under California's Confidentiality of Medical Information Act (CMIA) and other California laws, which can be stricter than HIPAA.

What this website collects

The appointment-request form on the Contact page collects only logistics: your name, phone number, preferred office, and the best time to reach you. It does not ask for symptoms, diagnoses, medications, or any clinical detail. Submissions go to a third-party form provider (separate from our electronic health record) and are used only to contact you to schedule.
